Our client is a large government department tasked with improving the lives of Australians. Our client is seeking an experienced Clinical Care Coordinator to join their team on a 2-year temporary assignment in Tasmania- various locations considered.
About the role:
- Deliver high-quality, evidence-informed clinical services including risk assessment, brief intervention, and care coordination for clients with complex needs
- Provide one-to-one care with a strong focus on client engagement and collaboration with families and caregivers where appropriate
- Support clients using structured approaches such as psychoeducation, distress tolerance, problem-solving, and motivational interviewing
- Maintain accurate and confidential client records, including case notes, care plans, and risk assessments
- Conduct regional intake assessments and support allocation to appropriate services
- Liaise with internal and external stakeholders, and community-based organisations
- Participate in case reviews, clinical supervision, and ongoing professional development activities
- Contribute to community development, service promotion, and program delivery (including group programs where required)
- Collaborate with clinical teams to support a stepped-care, recovery-oriented service model
About you:
- You will have previous experience in a clinical or counselling environment delivering trauma-informed, client-centred care
- You will have previous experience undertaking complex case management and risk
- You will have strong interpersonal skills, and the ability to communicate with clients and stakeholders at all levels
- You will have excellent written and verbal communication skills, and the ability to respond to critical incidents in a professional and timely manner
- You will have intermediate to advanced IT skills, and previous experience of using a clinical client or case management system.
Mandatory Requirements for the role:
- You must currently hold, or be willing to obtain, either Working with Vulnerable People (WWVP) registration, or Working with Children Check (WWCC)
- You must hold APHRA Registration as a Psychologist, Occupational Therapist
- Or you will be a Social Worker with membership of the Australian Association of Social Workers (AASW) upon commencement
- Or you will be a Counsellor with Master's-level counselling qualification with Registered Clinical Counsellor with PACFA, or ACA Level 3 or Level 4
Benefits:

- Full-time hours, Monday to Friday (7.5 hours per day generally between the hours of 8:30am and 5:00pm)

- $67.24 - $76.36 per hour + super
